    Nipple problems

    Nipple discharge
Nipple discharge is usually harmless and does not signify anything seriously wrong. In fact, most women can squeeze some discharge out of their nipples, especially if they have had children in the past. This may be whitish or may be yellow-green or almost black.

On the other hand, nipple discharge can be a symptom of breast cancer, particularly if it is bloodstained. Therefore, you should definitely discuss any nipple discharge with your doctor. If you are a man, you should see your doctor straight away, because the usual cause is a tumour and you will need treatment.

What your doctor will do. Your doctor will check your medication, because some drugs can cause (non-bloody) nipple discharge. The most common culprits are:

    • cimetidine (for stomach problems)
    • oral contraceptives
    • some antidepressants and other drugs for psychological problems
    • domperidone (for nausea).

Your doctor will ask whether there is any possibility that you could be pregnant. Some women have nipple discharge very early in pregnancy. Your doctor will then examine your whole breast thoroughly (not just the nipple), to make sure you have no lumps.

Even if you have not noticed any blood, your doctor may ask you to try to squeeze a few drops out, and will test it for microscopic blood.

Each nipple has about 15–20 tiny pores on it. These pores are the openings of ducts that connect with the glandular tissue in the breast. You and your doctor should try to work out whether the discharge is coming from just one pore, or from several.

    • The cause is very, very unlikely to be breast cancer if the discharge is coming from several pores, it does not contain any blood and you are under 50 years of age.
    • If the discharge is bloodstained, or it is emerging from just one pore, your doctor will refer you to a hospital clinic for tests (such as ultrasound, mammography and looking at the discharge under the microscope) to make sure that breast cancer is not responsible.
    • If the discharge is milky and coming from both breasts, your doctor can do a blood test to check for an imbalance of the hormone prolactin.

If all the tests are normal, you can stop worrying, but the discharge may still bother you (perhaps soiling your clothes). A possible cause is some inflammation (mastitis) around the ducts. This is linked with smoking, and may improve if you stop smoking and avoid squeezing. A course of antibiotics may help.

It is possible to have an operation to close or remove the ducts that the discharge is coming from. This operation may not be a good idea for anyone who plans to become pregnant afterwards – depending on the number of ducts involved, it might make breastfeeding difficult and the breast might become congested.

For more information about the breast, look at the section on breast problems

    Inverted nipples
Most women’s nipples protrude (stick out) about 5–10 mm. Usually they become about 10 mm longer and 2–3 mm wider during sexual arousal. Some women have nipples that are flat, but become erect during sexual arousal or when a baby is sucking on the nipple. Nipples that are tucked into the breast, instead of being flat or sticking out, are called inverted nipples. Both nipples may be inverted, or just one.

Nipples that have always been inverted. If your nipples have been inverted for as long as you can remember, it is nothing to worry about. It is just the way you are, and a lot of women are the same. A survey of 3000 women attending an antenatal clinic found that 10% had inverted nipples.

A nipple that suddenly becomes inverted can be a sign of a cancer underneath it, so you should see your doctor straight away.

Breastfeeding should not be too much of a problem if your nipples become erect in the cold or when you are sexually aroused. It will be harder for the baby to draw the nipple into the back of its mouth, so breastfeeding will require some patience, but eventually the baby’s strong sucking will draw the nipple out. You will be able to help by applying an ice cube wrapped in a flannel to the nipple beforehand and by stroking the areola (the pink area round the nipple).

If stimulation does not make the nipple protrude, breastfeeding may be more difficult. During the last 6–8 weeks of pregnancy, you may be able to encourage the nipples to stick out by wearing breast shields under your bra. These are small devices that press gently on the breast around the nipple. They are quite comfortable. They are worn for 1 or 2 hours at first, and the time is gradually increased. Your midwife will be able to advise you.

Ways of making your nipples protrude. Even if you do not intend to breastfeed you may wish to have protruding nipples. Teenagers often have flat nipples and, in some women (especially if their periods did not start until late), they remain flat until the early 20s. So if you are young, there is a possibility that they may gradually start to protrude. Otherwise, you could try stroking the areola with warm hands for a few minutes each day to bring the nipple out. You could also try wearing breast shields. Do not wear them for too long at first, otherwise the breasts may become sore, and do not continue wearing them for more than 6 weeks.

If these measures do not work and your nipples are really distressing you, it is possible to have a small operation to make the nipples protrude. This involves a small incision on each side of the nipple, and the cutting of some ducts and tissue. The drawback is that some women cannot breastfeed after this operation, and the operation is very expensive (and not available on the National Health Service).

    Hairy nipples
Coarse, dark hairs around the nipple are quite common. You can pull them out with tweezers, but this can cause irritation because the skin round the nipple is very sensitive. It is probably better simply to cut them off close to the skin. This is easier if you hold the end of the hair with tweezers to keep it taut. The hairs will grow again, so you will have to cut them off again from time to time.

Hairs round the nipple are nothing to worry about unless you have excess hairiness on other parts of the body and your periods are irregular. If this is the case, you may have polycystic ovary syndrome (PCOS) and you should see your doctor. You will find more information about PCOS in the section on hairiness in women.

    Itchy, scaly nipples
If your nipples are itchy, scaly and cracked, you probably have eczema. The itching can make this an embarrassing condition. It occurs mainly in women in their late teens, and usually affects both nipples. It may be only on the nipple, or may affect the flatter area surrounding the nipple (the areola). A steroid cream from your doctor will deal with it. Your doctor will check that it is not scabies, which can cause a similar appearance.

‘Jogger’s nipple’ is another possibility. This is caused by friction from clothing, especially during long-distance running. Protect your nipples with petroleum jelly (Vaseline) or surgical tape (Micropore, from pharmacies) before exercising. A silk running vest is less abrasive than synthetic fibres.

Very rarely, eczema of a nipple can signal a cancerous growth beneath. This is uncommon, but it is why you should see your doctor if you have eczema on only one nipple. It occurs mainly in middle-aged or elderly women. It is not usually itchy, but there may be a pricking or burning feeling.

    Extra nipples
Most people have only two nipples, but about 1 in 50 people (both males and females) has more. These extra nipples are not as well formed as the main nipples, so you may have them without realizing what they are. Imagine a line drawn between the armpit and the inner thigh; the extra nipples are usually on this line.

What is the underlieing stimulas for some womens' nipples to become visibly erect beneath their garments upon first meeting and during the ensuing conversation, when I personally am obese (vastly) and not at all attractive (in my own view)?

I am a user of prohormones. Through the cycle the estrogen level in males is low and testosterone is high. So when you come off the cycle the estrogen skyrockets and right now I have itchy puffy and tiny rash and this is a pre-case of gynoconmastia. Google it!

omg i am soo glad that i ahve read this article i feel much better now. after going on holiday there was damp in my bra and then it got all itchy and before i knew it all the top layer of skin had come of my nipples and they constantly ooze yellow liquid so i ahve to wear thick tissue pads inside my bra. I have had it go about 4 monthes now and have only just realised that it is probably eczema as i have had it since iwas born and it has gotten really bad recently. I use to just let it dry to stop the oozing but now i think im gonna apply my eczema cream to it and see if the oozing stops and my skin heals

I am seeing my doctor this morning and I plan to address my issue.

Both of my nipples get itchy, but it's mainly the left one, which also sometimes gets scaly (Just a very small patch of dry skin, but scaly is the closest description here.) as well.

I am bookmarking this page and I will let y'all know what he says.

P.S. I have tried over the counter remedies already. Everything from Cortaid, Monistat (in case of mild thrush) lotion, bath oils... etc. Hopefully today I'll get more of an answer.

Hi! well.. my problem is that my nipples are too big...my areolas are not big they're actually smaller than the average but the nipples are big and chubby..i've never been pregnant.

Posted by Realist at 27/12/2009 01:57:00

For all of you girls having problems, saying it's too embarassing to go to a doctor, please do.
Listen, if it is something serious, you would rather go to a doctor and be a little embarrassed then go on and find out it's something REALLY bad that you could have treated if you just went sooner.

You won't know until you go. It could be bad, or it could be not. At least you know and you're not wondering.
I hope this advice helped a little bit.

I have a VERY itchy nipple (left one). It's a LITTLE dry, and really only gets itchy when it's not encased in a bra. I don't know what's wrong, but I'm going to do myself a favor, and if I'm not pregnant, I'm going to a doctor.
    Please do the same ^.^
    You won't regret it.

    Hope it helped.

i have a lump growing in the middle of my nipple, the skin keeps peeling and it looks like it needs to be popped like a pimple, i was wondering if it has anything to do with my piercing, i am really concerned and would appreciate feedback i would hate it if i cant breastfeed in the future. Thanks

hey i have flat nipples, they become erect wen i masage the area around them but even then they protrude only a little and even that erection doesnt last for very long, they go back to being flat again quickly. I also have hair around my nipples and i pluck it out, but now i want to get rid of it. I cant tell anyone about it as it is embarrassing. Also the diameter of the dark area around my nipples is is big.

Please tell me that this all combined with the fact that i have a problem of multiple egg formation doesnt mean any serious trouble. I actually have a problem of formation of multiple eggs due to which i do not get my period naturally and take pills at times to get it. However sometimes my period does come naturally but that is very rare.

    Please help me out

My problem isn't listed here! I have these crust type things on my nipples (not on the aeola) I can pull them off carefully, but they just come back over time. It's really making me worry... I don't want to tell anyone I know, or go to a doctor because it's embarrassing... Please help me?

My left nipple is shaped pretty normally, but my right nipple is in the shape of a sideways v. It looks like it should be one nipple, but since it fold s inward in the middle (which is why it looks like a "v" - one side still connected) and comes to two "points" it seems like it's two nipples. It is always erect, while my other nipple only is when cold, etc. I'm only thirteen and to be honest I'm really scared. Is this normal? Do I tell my mom or a doctor?

What else can i do about it? And does anyone know what it could be? I am really scared x

i have a yellow spot around the outer nipple area on both breast what does it mean? should i be worry about breast cancer?

Posted by Anonymous at 11/11/2009 16:54:00

I have always had one flat or inverted nipple, it goes in when cold and I find this embarassing if I'm with my partner etc. I have been using a product called nipplette, which has helped a lot but not cured the problem, so I feel a bit more confident. It gets me down and wish I was normal, I feel like I'm the only one with this and unfortunately my daughter has both of hers inverted, I have thought about surgery, but I don't know if that will cure me and I don't know what other problems it could create. I've not met any other women who have this problem, it would be nice to know how they cope with it etc.

my nipples are painful to the touch and ooze out fluid. They feel lumpy to the touch.

I have been experiencing a dark green nipple discharge and was referred to the breast unit at my local hospital.
I was scared at the possible outcomes, but my doctor assured me that it was not dangerous with his final conclusion being that I have a problem with the Keratin in my body.
Keratin is found in skin, hair, teeth and bones. Basically, I do not produce strong Keratin - meaning that I get seepage through my skin. When I think about it I am constantly sweating, which could be a result of this too and my hair is always breaking and so are my teeth - coz I don’t have strong Keratin . I am about to embark on a course of Keratin tablets with my fingers crossed.

Hi, I just want to say it’s so good reading all these comments and realizing up not the only one with nipple problems! I have suffered from eczema since the age of 2; it was when I was 17 I started to get itchy nipples. I went to the doctor for the first time about it only recently and I’m now 24. I have been given a steroid cream which seems to be working. However because of years of itching the skin around my right nipple which has always been more prone now looks double the size off the other one. I am worried that the damage has been done. Will this ever sort it self out and return to normal size??

i'm a 21 y.o. male and my nipples have been extremely dry, itchy, cracked, etc. i get eczema on my hands occasionally but it goes away with creme. it doesnt go away on my nipples. i saw a dr. once and got an ultrasound (mammogram would be difficult on me) and nothing came of it. that was a while ago though and the fact that i still have this problem is somewhat concerning. wondering if i should see a dr. again?

My left nipple has been extremely itchy recently and I've never experienced this before. I'm reluctant to seek help though especially as I've only seen my current gp a few times. Any ideas for natural remedies??

Hi, ive always had invtered nipples, i'm really embarrassed about them and now that i have a boyfriend i'm really scared to tell him about them because he might tell other people. Should i go to the doctors about it? I read that the surgery was expensive, how much do you think it is?

I was wandering if someone may be able to give me some advise please?
During the last few months - roughly since Christmas - I have noticed that occassionally the nipple on my right breast becomes inverted for short periods of time.
I breastfed my son for a year but stopped when this started happening.
As of yet I havent seen my GP as I have no other symptoms, although I am still concerned.
    Many thanks for your time.

And when rubbed by clothes ,can become like a red bubble that hurts and eventually goes back down. But it doesn't seem to be reattaching to my nipple,I don't have insurance and am unable to get it checked just curious if you have any information thatmight help me thank u for listening! - Kelly

I have had an inverted nipple for as long as I can remember and every now and again it can produce a smelly yellow discharge if I squeeze it - is this worrying at all?

Posted by Anonymous at 03/04/2009 06:55:00

I have had a milky discharge coming from my left breast and from one nipple duct only. Could this be a sign of breast cancer??

I have an inverted nipple that has happened recently. I also have pain sometimes in the outer part of the same breast. Along with my breast itches. My doctor did a sonagram, mammogram, and a bsgi. They all came back pretty normal. What should I do about the pain and itching. My dr said wait three months and retest....but three months with an itchy painful breast??

I found the information "Itchy, scaly nipples" very helpful!
This is what my doctor diagnosed a few months ago, & with the help of the cream given, I have seen a dramatic change colour & not itchy anymore,
    Cheers for the info!

I have one nipple that is inverted 95% of the time but I am able to push on the sides of it and it becomes everted. Is this ok? do I need to get a 2nd opinion? I have regular digital mamagrams and they all come back negative. I'm afraid they are not reading them corectly??

I am a 23 yr old female. My nipples are "VERY" flat and always have been. I've read that there is an expensive surgery that can fix this problem, but I do not have any health insurance or the money to pay for the procedure. This has always bothered me especially during sex. I've never wanted my partner to touch or see my breasts because of my nipples being flat. I wanted to know if there is any medicine that I can take or any other method of fixing this problem that will help me. Thanks

it is embarassing to talk about. The advice that was given is helpful. I have had a child and didn't breastfeed. I missed that experience and want to try for a second child. I know it will be harder, but i can do it. Thank for the helpful tips.

If your nipple is inverted as was not prior, YOU MUST SEE A DOCTOR. It could be a sign of breast cancer, which is very treatable if caught early.
