    Bacterial vaginosis

    ‘Bacterial vaginosis’ is a very common condition in women. In fact, it is much more common than thrush. In the UK in 2007, more than 92,000 people attended a clinic because of bacterial vaginosis. If your genital area smells fishy, this is almost certainly the cause. It is an imbalance of the bacteria in the vagina. Its previous name was ‘anaerobic vaginosis’, and some medical textbooks still use this term.

    What is bacterial vaginosis?
    Every woman has many varieties of harmless bacteria in her vagina. In bacterial vaginosis, some of the bacteria multiply too much, so that more are present than is normal (especially Gardnerella and Mobiluncus bacteria) New England Journal of Medicine 2005;353:1899–911). And the numbers of other friendly bacteria, especially the ‘lactobacilli’ type, are decreased. In other words, bacterial vaginosis is not an infection caught from your partner – it is due to bacteria that are normally present in the vagina.

    Symptoms of bacterial vaginosis

    • The main symptom is a fishy smell in the genital area. You may notice that the smell is worse after sex and during your period.
    • There is usually a discharge, which is watery and greyish-white in colour.
    • Bacterial vaginosis does not cause soreness or irritation.

    If untreated, bacterial vaginosis may possibly increase the risk of pelvic inflammatory disease (infection of the Fallopian tubes that lead from the ovaries to the uterus).

    Why women get bacterial vaginosis
    Putting antiseptic in your bath water, or using bubble baths, can make a bacterial imbalance more likely. Another cause is douching (squirting antiseptic or soapy liquid into the vagina). Some women douche the vagina because they think it is hygienic, but the opposite is true. The vagina cleans itself very effectively and douching makes infections more likely. Some research suggest that bacterial vaginosis is more likely in women who smoke, or who eat a high-fat diet (Journal of Nutritrion 2007;137:2128–33).

    What you should do
    If you think you have bacterial vaginosis, you need to see your doctor because the treatment, an antibiotic called metronidazole, is available only with a doctor’s prescription. Metronidazole cures the problem in 90% of women, but causes an unpleasant metallic taste in the mouth, though this disappears after the treatment is finished. It can also cause a slightly nauseous feeling. You should not drink alcohol while you are taking it. Metronidazole is usually prescribed as a tablet taken by mouth, but a gel form that is inserted into the vagina is sometimes used. Clindamycin vaginal cream is another possibility.

    If it comes back. Unfortunately, the cure may not be permanent. The symptoms return in about half of women. In this situation, there are various possible courses of action.

    • See your doctor again for a repeat of the metronidazole treatment, or to try another antibiotic. Make sure you take all the prescribed medication; a research study has found that bacterial vaginosis is most likely to come back in women who did not take the whole course of antibiotic treatment (Annals of Internal Medicine 2008;149:20–28).
    • Try acetic acid vaginal jelly, which you can buy from a pharmacy without a prescription (such as Aci-Jel or Balance Activ). These restore the natural acidity of the vagina, which may encourage a return to the natural balance of bacteria. It is not known whether or not they damage condoms and contraceptive diaphragms.
    • Try yoghurt. Make sure the container says ‘live yoghurt’, because this contains live lactobacilli bacteria. It is the friendly lactobacilli bacteria that are reduced in numbers in bacterial vaginosis. Gently smear a small amount of yoghurt over the vulva (the area around the opening of the vagina), and also put some inside the vagina. The easiest way to do this is to use a tampon with its applicator. Push the tampon back inside the applicator so you have a space for about a teaspoonful of yoghurt. Then insert the tampon in the usual way, which will push the yoghurt into the top of the vagina. Remove the tampon an hour later. Do this twice a day for a week. According to the medical journal Bandolier, which looks at the scientific evidence for treatments, some research has shown that women using yoghurt treatment felt their symptoms were improved (www.jr2.ox.ac.uk/bandolier/band60/b60-3.html).
    • If you have an intrauterine contraceptive device (‘coil’) and bacterial vaginosis is very troublesome, consider having the device removed. Bacterial vaginosis is more common in women using this type of contraception.
    • Stop smoking, as smoking seems to be linked with bacterial vaginosis.

    If you have a partner. Your partner does not need to be treated, even if you keep getting bacterial vaginosis. There is no scientific evidence to suggest that treating your partner makes a difference.

    If you are planning pregnancy. In the past, bacterial vaginosis was thought to be just a nuisance, but not harmful in any way. There is now evidence that it doubles the likelihood of a premature birth. So if you are intending to become pregnant, you should have bacterial vaginosis treated beforehand. If you have had premature labor in the past, your doctor will probably test you for anaerobic vaginosis during your pregnancy and treat you with antibiotics if necessary (Annals of Internal Medicine;148:220–33).

    Hello everyone. On reading this very useful information on bv I decided to share my story. I have been suffering on and off with bv for about a year and a half now. I have had four courses of metronitozole 2 aplicators of the clyndomycin and tried an anti biotic called tinidazole.... yet it comes back. It gets me down so much because when it flares up I cannot even sit on a seat without infecting it with stench. How embarrassing! Fed up I returned to me gp and he is now referring me to a gynocologist. I cant wait and hope and pray they can help me. I plan to try the natural 'live' yoghurt as this is knew advice. will let you know how I get on.

    so, you guys tell me what you think.

    i was in a relationship with a guy. lots and lots of sex. then all of sudden BV. i got both of us treated with oral antibiotics. it went away and we broke up. a few weeks later, we were back together. sex- well yea. of course, BV back in action. he admitted that his ex had it at some point. (just what every girl wants hear). then he tries to tell me that he thinks it must be in his mouth. we both go back through round 2 of antibiotics.

    i got rid of him but have always wondered if maybe a guy can have this @$%^ growing in the mouth. anyone ever heard this? any doctors on that might have thought? mine pushed me off to his nurse who was so floored i asked that i never got an answer.

    I hve been experiencing a problem with my discharge. It has an unpleasant smell and a yellow greenish colour, sometimes brownish.I eve experience pain during and after sexul intercourse. I sometimes bleed after sex.I WAS ON TREATMENT AND THE SYMPTOMS DISSAPEARED BUT NOW THEY ARE BACK AGAIN WHAT SHOULD I DO?

    I keep getting bacterial infections and I go through with the medication, it gets better but then comes back a few weeks later. Do I have to keep going to the dr to get it treated? How do I keep it from coming back, is there anything I can do?

    I have had sex recently and noticed my partner had a fishy like smell down below recently I have showered using a perfumed bodywash ibecame irrated not sure if this is connected but my vagina had a fishy smell days later.my vagina has never ever before smelled this way.what can this possibly be there are no symptoms such as burning or itching

    hi, i have been getting discharge for as long as i can remember(3-4 years anyway) it's quite thick and it does smell quite bad, especially when i do sport and become hot. i started my periods in april last year but i still get discharge. have i got BV?

    I have symptoms which seem to refer exactly with BV, however the discharge is greenish in colour and has the consistancy of cottage cheese, could this be another infrection or another symptom of BV?

    hiya, im fourteen and in the past two or three days i have had a clearish/light yellow discharge that smells.. not of fish just unpleasant. i hav started my period but not had it yet this month, do i have BV or is it just because my periods coming? please please help im worried. :|

    i'm only 13 and i've noticed that recently i've been getting this wierd sticky white stuff whenever i go to the bathroom... i got my period for the first time about 4 months ago. I really don't want to go to the doctor and i don't want my mum to know either... i really need some help on what i should do, any conspicuous treatments or foods or anything?? please i just need some help!!

    I got back from the GUM clinic today and have been diagnosed with BV and also a urine infection. This is the first time i've had it. I've been ill with chest/throat infection for the last week or so and have been on amoxicillin since Thursday. On Friday i noticed pain when urinating and some vaginal redness - i have previously had thrush caused by antibiotics but this was definitely not the same!! I have had a change in discharge - discoloured (slightly yellowish?) but doesn't really smell. I was terrified that i had herpes or something terrible so am somewhat relieved to find it is only bv.
    If you have any of these symptoms just go the GUM clinic and get it checked. I must admit the examination was pretty painful - inserting the spectulum (?)was very uncomfortable due to the inflammation etc. I've been given 5 metronidazole tablets all of which i must take tonight. Hopefully this will resolve the problem. I have since noticed a couple of red bumps on my outer labia but am hoping this is just due to general irritation/infection in the area rather than anything more sinister...

    Hi all sis commarades..

    I been recently diagnosis with BV after a visit to a gayne. The results can be known. (where the doc take a cotton bud wipe on the liquid sample from ur vagina sent to the lab for testing).

    In fact I didnt know I had it. Was doing the normal cervival test - pap smear test that the result shows abnormal thus going into it further to check.

    The doc prescribe me something to put into the vagina for 3 days supposely to clear it. Anyway went for my pap smear again..hopefully normal results this time round.

    Anyway dear sis around, pls consult gayne who r more trained in woman body then normal doctors. They will suggest the several test etc to confirm n treat... Dun live in fear n worry....

    One more thing......your doctor may or may not tell you that antibiotics will kill the good bacteria in our bodies, causing an imbalance in our system. Take acidophilus to prevent this! :)

    Drinking acidophilus milk worked for me! You can also buy acidophilus pills in the health food stores (may be available elsewhere by now). I have yet to run into a doctor that suggests this very easy, and drug free, solution!

    i am 15 an im a vergin but i'm really worried because i'm not sure if i might have bv or thrush or something.
    it's quite itchy an i have a white quite thick dischange an it smells quite fishey. it's really worrying me and i don't want to tell anyone or go and see a doctor about it.

    i have got bv i have got to take 2 tablets twice a day for seven days, this is the second time i have had it, the first time i had to take five tablets all at once so i can not understand why they have gave me a weeks worth of tablets this time as they said it was a slight infection, i did have a clear discharge and the only time i had a bad smell was after intercourse, but i am getting a lot of pain in my pelvic area and it is also painfull some times during intercourse, and iam under the hospital as they have found cysts on my overies so now iam wondering if this is all related to bv. does anyone else relate to any of these other symptoms, if anyone does could you give me any advise on this please

    My periods have always been very regular, last month it was 2 weeks late when i went to the doc and was told i had bacterial vaginosis. I am taking anti biotics and again my period is due. Can the BV affect my periods in any way as i am now concerned that my 'period problem' is something more sinister. Blood tests have shown nothing wrong.

    I have a bacterial vaginal infection and I have been to three doctor. One doctor said it was Trick. The other said it was a bacterial infection.But, I have discharge and no itching or burning. Which doctor is right? Is it hard to tell Trick from a bacterial vaginal infection.

    I had called my obgyn a while ago and told her that i have a clear watery discharge it does have a smell but i cant quite describe it and she told me that it is normal i have gotten checked for diseases and everything came back negative.. from what i have read up-top i'm not sure if that has any thing to do with BV cause they talk about the discharge being a watery grayish-whit color soo i was wondering if someone could help me by letting me know wht the problem is (by the way it is kind of hard to tell if it's a grayish-white color becauseof the white toiltpaper i use)PLEASE PLEASE LET ME KNO IF IT IS BV I WOULD REALLY APPRECIATE IT THANKS

    I had just gotten over a yeast infection and was excited to be back in bed with my partner when he noticed the smell. Embarrassing! I thought it might have been the treatment but seeing this. I think if it doesn't go away in a few days I’ll try the treatment for BV

    I've been dealing with BV for over a year now. I am 26 years old, I eat a healthy vegetarian diet, I have no children and have only had sex with 1 man my entire life. I don't douche, and I wipe myself the "right" way and wash regularly. Why is this happening to me? Sex is getting to be uncomfortable and embarrassing because of the smell. I was given some pills and it cleared up for awhile, but now the smell is coming back. What can I do?

    i slept with a bloke who gave me clyamidia. After i got that treated. I got this smell. i thought it was jus the antibiotics and it would go away. Its not a wrank smell but i get parionoid that everyone else can smell me aswell! so i thought id have a look to see what it could possibly be and now i know!

    I've been researching online to try find out what may be wrong with and read about BV which I'm convinced is the problem but the smell is certainly not worse after sex, quite the opposite. Does anyone else have the same?

    i have been diagnosed by my doctor that i have BV. She gave me 2 capsules, i took them, but i am not sure if it was fully recovered, because i took the first capsule not with an exact way, n when i went back to my pharmacy, they didn't want to sell me the capsules. Thanks for this website, i will try the acetic acid vaginal jelly, to make sure that it is fully recovered and doesn't come back again.

    l have been diagnosed with bv, and iam still breastfeedinding my baby, so the dr. does not want me to take the regular antibiotics.and has prescibed a biodine deuche. she says this is what women always did in the old days. unfortunitly i cannot find this stuff at all.iwent back to th dr. she was very surprised iwas having a hard time getting this stuff.andshe thought maybe the icould find it on line. icant . its a real pain i have this since sept.

    For a few years now I have struggled with vaginal discharge and smell. My doctor says I am normal, but I beg to differ. I feel that I can have an odor and although this may sound silly...I have tasted myself and feel that I taste bitter. I'm not sure if I am just being paranoid because of the emotional abuse by my ex-partner or if I actually have a problem. I did have bacterial vaginosis re-occur for quite awhile, but was just tested and it was negative. I also find that the smell is unpleasent after I have sex with a new partner.

    i think i have BV as I have a strange smelling discharge although not quite fishy. However, I also think it may be thrush as I have some cracking and the entrance has been itchy. I will try a course of thrush treatment and see how I get on

    I have developed BV after having the coil fitted,although I also smoke and eat alot of fatty foods and wash twice a day with soap free products.Glad I have found out what the possible causes are as the fishy smell is so embarrassing especially after sex.Was pleased to know that BV is not a sexually transmitted disease.After taking my course of antibiotics I will definitely look at changing my diet and try to quit smoking for 2009.
