    Anal pain

    Pain coming from the bottom area is not something that we are used to feeling and, as a consequence, anal pain can cause a lot of worries.

    What sort of pain is it?

    • A knife-like pain when you have your bowels open, and which may last for 10–15 minutes afterwards, is probably caused by an anal fissure. Some people describe it as like ‘passing glass’. In addition to the pain, you may notice some bright red blood on the toilet paper.
    • A similar knife-like pain can be caused by herpes simplex virus.
    • A nagging, aching discomfort made worse by defecation could be due to piles.
    • A throbbing pain, worsening over a few days, and bad enough to disturb your sleep, is likely to be caused by an abscess.
    • An occasional, severe, cramp-like pain deep in the anal canal, lasting about half an hour, is probably a condition called proctalgia fugax.
    • A continuous aching pain in the anus needs to be sorted out by your doctor. It is most often caused by a back problem (when a part of the spine presses on a nerve).

    Anal fissure
    An anal fissure is a split in the anal skin, just inside the anus, usually towards the back. The pain of an anal fissure feels like being cut with glass when you have your bowels open and afterwards. It is worse when you have had a hard bowel motion. You may notice some bright red blood on the lavatory paper at the same time. You may be able to feel a small lump alongside the crack; this is a ‘skin tag’.

    Anal fissures are most common in teenagers and young adults, and there has often been a period of constipation beforehand. Sometimes they follow childbirth. They usually heal on their own, but this often takes several weeks and the scar may split again.

    What you can do. Do not feel anxious about the fissure. Although it is horribly painful, it is not a dangerous condition.

    • Avoid constipation – this is very important. Keep your faeces soft by eating lots of bran cereals, fruit and vegetables.
    • Smear some painkilling gel around the area just before you have your bowels open. You can buy a suitable painkilling gel from a pharmacy; they are sometimes labelled ‘for piles’ or ‘for haemorrhoids’. It will prevent the anus going into a spasm, which can make the problem worse. Do not use the gel at any other time, or for more than 1 week, because you can easily develop an allergy to its ingredients. If you notice soreness and itching, in addition to the sharp pain, it is quite likely that the gel is responsible.
    • Warm baths twice daily can help, if you have the time. Put a handful of ordinary kitchen salt into the bath.

    How your doctor can help. You should see your doctor if the problem is not improving after 3 weeks. If you feel anxious about this, look at the section on seeing your doctor about an anal problem. For some treatments, your doctor will need to send you to the ‘rectal clinic’ at your local hospital. Some of the treatments your doctor may use are listed below.

    • Glyceryl trinitrate ointment, applied several times a day for 6 weeks, seems to heal the fissure in 60–70% of cases. However, about 50% of the people using this treatment get a headache as a side effect.
    • A gel containing the drug diltiazem is being tried in some hospitals. It can help people who do not respond to glyceryl trinitrate.
    • Some specialist hospitals have been using injections of botulinum toxin (Botox) into the muscle of the anus. Botulinum toxin prevents spasm of the muscle, because it blocks transmission of nerve impulses to the muscle. This treatment is not widely available. Healing rates are about 69–90%, but it needs more research to see how effective it is in the long term.
    • The most common treatment (before glyceryl trinitrate ointment came on the scene) used to be a small operation under a general anaesthetic. The operation is designed to overcome the spasm of the anal muscle. The pain relief is dramatic and instantaneous. In the past, this operation could leave you less able to control wind, but a new type of operation (tailored sphincterotomy) does not have this drawback (British Journal of Surgery 2005;92:403–8).

    Unfortunately, the fissure comes back within 3–4 months in about one-third of people treated with glyceryl trinitrate or diltiazem, and in about 40% of people after treatment with botulinum toxin (Gastroenterology 2002;123:112–7). Surgery seems to be the best long-term cure and is effective in about 90% of cases.

    Herpesvirus infection
    A herpesvirus infection can produce a pain similar to an anal fissure. Herpes can infect the anal area, either spread by the hands from a cold sore on the face, or transmitted as a sexual infection. At the anus, herpes often forms a crack rather than the small ulcers that tend to occur elsewhere. It can occur in individuals who have never had herpes elsewhere. The soreness occurs in episodes, each lasting for a few days. A genitourinary medicine clinic will be able to take a swab to check for the virus if you visit the clinic as soon as an episode starts; if you are worried about the thought of visiting a clinic, take a look at the section on visiting the clinic.

    Abscess
    An abscess close to the anus produces a throbbing pain that worsens over a few days, and is usually bad enough to disturb your sleep. You may be able to feel a tender swelling in the skin beside the anus, or the abscess may be hidden inside. This is unlikely to go away on its own; it needs to be lanced by a doctor.

    Proctalgia fugax
    Proctalgia fugax is a severe, cramp-like pain, deep in the anal canal. It usually lasts for less than a minute, but can sometimes last for up to half an hour. Most sufferers have only 5 or 6 attacks a year. You may feel a need to defecate urgently, but nothing happens. It may even make you feel dizzy, or give you a headache. It occurs in both men and women. The pain often wakes sufferers at night, and men may have an erection at the same time. Some men experience it after sex. It is a mysterious condition; no one knows what causes it, but it is probably a spasm of the rectal or pelvic floor muscles and does not mean that you have anything seriously wrong. There are various methods of relieving the pain.

    • Try putting pressure on the perineum (the area between the back passage and the vagina or base of the penis) by sitting on the edge of your bath or on a tennis ball.
    • Sit in hot water or, alternatively, apply some ice.
    • Two paracetamol tablets and a hot drink may give some relief.

    The problem with medications for proctalgia fugax is that the episode is likely to be over before the drugs become active. They might be worth trying if your attacks last a long time. Possible treatments are glyceryl trinitrate spray or under-the-tongue tablets (as used for angina), glyceryl trinitrate cream applied to the anal area, or the asthma drug salbutamol (inhaled from a puffer at the start of the attack). These treatments are only available on prescription, for which you would need to see your doctor.

    i feel pain from the anus opening to my gut and back , i feel as if i'm in labor and i'm passing neddles ,i have to grasp on to the walls in the bathroom , can't speak , hard to breath , and it's so painful ,

    i cant really find anything to what is wrong with me, i been having a stinging pain there for the past day or two, it wasnt that bad, i kept trying to get comfortable but today i felt a big oval shaped lump, when i checked there was something black under a thin layer of skin,it will randomly strat a painful stinging, its freaking me out and i dont know what it is, if anybody has had this plz tell me what it is.

    For proctalgia fugax pain... mine lasted for an hour or more at times.. My Urologist provided a script of Darvoset to ride out the pain which is very intense... rivals kidney stones... I did not want to use the pain relievers ... I have now found that using a vibrator or vibrating massager helped tremendously to relax the muscle in question... The Urologist likened it to a charley horse in the spinchter muscle...

    i had anal sex for the first time with my boyfriend. it did hurt at first but he used some lubricant and it felt better. now i have some kind of sting in my anus and even i have back pains. i want to know what's going on.

    i have had a few problems with going to the loo constipation and havnt gone at all for a few days, i have had to resort to taking senokot, i now have found that i have got an ache that is there all the time which is inside my bottom area

    Proctalgia Fugax. I have suffered since the age of 12. I find that a snack , piece of fruit or a drink almost immediatley reduces the pain.

    I suffer from proctalgia fugax sometimes. It has occurred fairly regularly since I was mid 50's.

    I am 65 and still suffer, on occasion, from it. Normally I am very regular as far as passing solid. But yesterday I passed a fairly small amount of faeces and late last night had to pass more. It was somewhat forced and resulted in an a attack of proctalgia fugax some 30 minutes later.

    These attacks do last more than 30 minutes and I should try some of the remedies suggested. But it is difficult to consider the remedies after some time asleep. I have to say I have had a good healthy life and find this problem to be mystifying.

    There is a slight nag of pain throughout the next day but does go away. It is easy to forget about the
    problem, until the next episode.

    I am relived this problem seems not to indicate I have anything seriously wrong. However it is a severe pain and I can suffer severe pain without it being obvious, but this is different. I have decided to get out of bed and let my wife sleep in peace.

    Ive been to the doctors about this stabbing pain repeatedly & now im starting to get abdominal pains with it, probably because its too painful to open my bowels, I have to use stool softeners daily & sitting on anything hard makes the pain a whole lot worse! Have any ladies also suffered during sexual intercourse with this pain has I literally passed out from the pain after making love! Now ive been refered to gyneacology too! Am I the only one suffering like this??

    I am just relieved to find a name for the anal cramps I have had off and on for nearly ten years. Proctalgia fugax. And to find that others can comiserate. I have had two children that weighed over 9 pounds without any form of pain reliever, but the "butt pain" can bring me to my knees. I find that drinking a great deal of hot water upon onset can lessen the severity. It sounds silly, but it really does help, wether it is psychological or not. Thanks to all of you who have been honest enough to admit your pain.

    i have an irritating itch in my anus i have been to the doctor several times and all they gives me is creams that dont seem to work and sometimes seem to making the area feel like its burning (on fire really!)it keeps me up at night and even early hours of the morning cause it irritates me that much and it sometimes bleeds wen i been screatching it or occassionally after beeing to the toilet i have had this problem for years and have tried so many creams. it irritates if i wipe it with warm water

    Anal are is like lips around the opening and swell when I try to push the stool out. Its like the sphincture muscle won't push it out or realease. I push on the swollen area and have to help the stool out.

    I think doctors should just cut the hemroid off. That way people don't have to go back and forth to the hospital about the same reason. Sitting in a hot bath, ointment product, stuffing the hemroid back in is unnessasary. But the doctors make a huge laugh about peoples hemroid hanging out there butt is insane.
    I just wanted to tell everybody with hemroids eat food with fiber and exercise. Drink alot of water eat fruit and veggies and salad. You can go to the store and but metamucil for ten dollars it hac alot of fiber just add with water drink it after you eat so you won't have hemroids.

    I think I have experienced the symptoms of Proctalgia fugax. I find moving about or adjusting the position of my hips usually makes the pain go away.

    I've been to the doctor several times and the emergence room once since the problem started back in Jan and has not went away i eat more fiber take more stool softners than the ever and nothing seems to work. After a bowl movement i am in the worst pain of my life I can't sit I can't stand lieing down doesnt even work a hot bath works ocationally but most of the time I'm work and can't think straight is there any kinds of test the doctors should do to figure out what the problem is and get it fixed.

    ive been having what feels like sever cramps or stabbing pain, i think either in my anus or in my intestines, im not sure, i feels like a knife being twisted and nearly brings a tear to my eye its so painfull, only lasts for 30 seconds at the most and have occured 1 - 2 times a day for about a week. what is it? it hurts!

    i have a "burning" pain in my anus which is constant .i do not suffer with piles,i eat a vared diet.also i have no problems opening my bowles.i have been told i have prostrate caner,and i am being treated for this with:injections and hormone treatment.i cannot be cured for this as this cancer is to progressed to remove. can you give me some advice? thanks ken

    It appears that my problem is what is described as Proctalgia Fugax. It is a painful and unpleasant experience. I find relief from using suppositories called Arthrexin which is fairly expensive in S Africa. I have not tried anything else as Arthrexin gives relief and I keep an ample supply. Fortunately it occurs once or twice a month as your article indicates.The cramps have also occured within a day of each other, and I have also had these cramps during the day. It can be most embarassing, especially if you are in the company of your colleagues or boss, and it takes a lot of concentration to process the situation. I have wondered whether it may not be caused by a calcium or some other mineral deficiency.

    I have been having short, stabbing pains in what feels to be my anus for a couple of days now - I have experienced it before, but then they seemed to disappear for a couple of weeks before returning! They last 2-3 seconds roughly, and are incredibly painful!! Does anyone know what this is likely to be?? I am very worried about this!! Please help in any way you can! Thanks....

    Five months ago my primary care expressed the 'head' of my anal abcess which had burst earlier. I was given an antibiotic and sat in a sitz bath 4 times daily. I then developed cystitis which I believed came from 'fowl' water sitz. I walk the floor, can't sleep because the constant 'abcess like' pain, burn, itch has returned. This site is correct in warning not to use OTC anal remedies for extended periods. All problems began with my using them for months to clear an external hemmorhoid, which I did. I have soft stool 3 times a day, requiring lots of wiping. Now I have brought my problem back. M wondering if a bidet instead of t.paper could stop this recurring agony?? I found MSM and Olive Leaf Extract helpful in between reducing inflamation. I have not had sex for over 10 yrs. w/divorce. Being 60 I wonder if possible prolapse from large babies is behind it all.

    This sounds quite like the pain I sometimes have in the anus. I've often wondered what it is. In my case, I feel it is caused by emotional stress and tension earlier in the day.

    Ayurveda has a cure. I had thrombosed piles and the doctor suggested lancing. I chose to take auyvedic medicines. With no operation,with ayurveda treatment the pile disappeared in 2 weeks.

    I have LS which seems to be under control however I still have a problem going to the toilet which could be connected to the LS.My feces are very small almost the size of thick string and unless I take some sort of laxative that softens the stool I struggle to go. Several times this has caused anal fissure with severe pain. Can the anal canal shrink and get tighter. My doctor does not seem to see the problem with my anal as being as important as the LS around the vagine area

    Hi, I have an anal fissure and doctor prescibed me glyceryl trinitrate ointment but I get a headache for a few minutes after using it. My consulatation is in four weeks time and I do not know if I have to stop using it or should I continue and do not care about side effects???

    I had this anal pain when I was in my mid forties --- there was stress, but not that great. Doctor could find nothing wrong and gave me a relaxant.Eventually pain went away, and only now recurs --- I am 73, and diabetic last two years. I think that it is related to the leg cramps I have been having at night --- which are a result of diabetes, I am told. When I take magnesium, the night cramps go away. I will try taking it for rectal cramps now. Also, I understand that peripheral neuropathy is a result of diabetes ---and although I do not (yet!) have it, this might be a precursor. Finally, the pain is one sided and shoots into the hip and leg -- so bad I can barely stand up -- but once I stand and rebalance, it seems to go away. This is not arthritis (per MD), it is sciatica, I think MD said. I didn't ask more info -- being relieved it wasn;t worse diagnosos.

    I am bisexual leaning gay, I had IBS for 3 years in which I had bad diarrhea all the while, when I would have a bowel movement it would burn so bad I wanted to pass out. I no longer have IBS but my bottom never recovered completely, I want to engage in anal sex but anything inserted is too painful to be enjoyed, The pain is right inside my opening, and Ive been to the doctor and he said there are no hemorrhoids, The pain feels like my anal walls are just raw. PLEASE HELP!!!

    Ive had these cramps for 8 years now around twice a month in the middle of the night. They last around half an hour and I feel sick. I find walking around helps. I have been worrying what it may be but reassuring to read that it isnt usually serious and that other people have it too!

    I got a anal fissure. The skin outside the anal is torn out . I used 2 apply vasline when it was sore. Now its worse coz of itching . I got this problem because i went for polynoscaphy

    I have experienced anal pain from childhood. when I read above i.e.Proctalgia fugax , I think it is the reason. The pain usually lasts up to 5 minutes but wakes me up in the night. it is once or twice in a month.I do not know how to prevent it.

    i have a sharp stabbing pain in my stomach about 4 inches up and an inch and a half left of my belly button. The odd thing is that the only time i feel it is when there is pressure placed on my anus, such as wiping after a bowel movement.

    I have had this unbearable pain which is called by my surgeon, anal fissure, for 2 month. Every day,5 to 6 hours after passing stool. I see no blood.I tried all creams & pain killer, and nothing helped. Then 3 days ago I went to the operating room and had a incision. I am suffering even more!! Just pain. Any help?

    Sometimes when i go to the toilet, and i strain or it doesnt feel very easy to pass there is blood in the toilet after, and on the toilet tissue. its bright red, however it stops imediatley. does this sound like a fissure?! i have no pains etc other than the pain i get when i try to pass... which is stinging.

    I attended the GP due to pus and mucous on the toilet roll also with some blood. Upon examination he said I had a large fissure which was oozing pus. He has referred me to the hospital to investigate further. However i started slimming world at the start of January and since eating ALOT of fresh meals and cutting way down on bread its not as painful, there is little mucous and the ache has gone. Will keep you posted. X

    I find this page very useful; it has addressed some of those frustrating anal problems. However, i'm experiencing an anal problem which seems to be different from those discussed on this page. The anal pain i'm experiencing started as a result of colon detox. It was a natural detox exercise i underwent. That was fasting and drinking only water for 2 days, then after the two days, i ate only watermelon, which was also a kind of detoxifying agent. The watermelon really eased my bowel movement. So this pain came as a result of my frequent visit to the toilet after eating the watermelon and drinking only water, which i also did for 2 days, preceeding the 2 days fasting. It looks like my inner anus has been pushed from its natural position and needs to fastened back; that's my feeling, i'm sure. I feel pains when the anus contracts, but no pains when it expands or closes. Any help?

    i have these pains in my anus, deep in side and sometimes in the area between my anus and testicles. The pain comes and goes away. Tow days ago, I have seen my GP and he told me, after the anus exam, that there is nothing serious, but told me i need to do a prostate test ( give my blood) I am extremly worried, can anyone help?     I have had this excrutiating pain deep in my unus for more that a decade now.The pain then goes through the penis and makes ejaculation very painful.I now have a permanent pain through the penis and I see colourless discharge like semen coming out of the penis.It is very painful.I dont kow if this is infectious as my wife also has a continous discharge(white)

    I also see a wound next to the unus opening.I tried to treat it with diferent medication but it does not get right.It is also very painful.Please answer me as I have gone to many docters but I do not find cure

    It has been experiencely felt that mostly disorders relating to large or small intestines,rectum,annal fissures fistulas ,piles and abcess etc.relate to constipation hence in this regard it is proposed that the patients should always remain in try to free of constipation by doing proer physical exercise and taking isphagol husk so as to keep thier stool in soft condition

    I hm have an incnvinient problem that is aching my anal. It had happened since my childhood. I had many treantments that made it disappear for many years or decades, but it appears again.
